The Joint Commission on Hospital Accreditation, each year publishes data on a “sentinel event”, a patient safety event that results in death, permanent harm, or severe temporary harm which is preventable.
The Joint Commission Sentinel Event Data 2023 Annual Review again reports record high levels of patient care errors.
